characteristic that you believe has effected your employment, (f) Date of Termination, (g) any leaves
you have taken and the reasons for them, (h) your employer's reasons for termination, (i) why you
believe you have been terminated, (j) whether you are aware of an agreement to limit the statute of
limitations (k) whether you are an at-will or just cause employee.
If you are alleging illegal harassment, please state when it started, to whom you have reported it, any
company response to your complaint, who the harasser is (i.e. co-worker or supervisor) and the
details of the harassment.
For criminal matters, please (a) provide the nature of the allegations, (b) whether formal charges are
pending, and (c) in what city/county/court your case is pending.
For all other matters, please provide a brief explanation of your matter, including whether court
proceedings have been initiated, in what court, and what relief you are seeking.
